What You'll Do
Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
Collaborate for Optimal Outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.
What You Need
Valid California Physical Therapist license
CPR or BLS certification
Driver’s license and auto insurance
Physical + TB test (within the last year)
Professional liability and business license (if applicable)
