What You'll Do
- Provide Skilled MSW Care: Conduct in-home assessments to evaluate geriatric patients' living situations, support systems, and resources.
- Secure Vital Resources: Connect patients to community aid, such as IHSS, Meals on Wheels, and other programs that address their social determinants of health.
- Assist with IHSS: Guide patients and families through the IHSS application and modification processes.
- Collaborate with the Care Team: Work with other disciplines and administrative staff to ensure a comprehensive, coordinated approach to patient care.
What You Need
A valid California LCSW or MSW license
Home health experience is a must.
CPR or BLS certification
Driver’s license and auto insurance
Physical + TB test (within the last year)
Professional liability and business license (if applicable)
