What is our service?
LifeSpan Physical Therapy Services, P.C. is an onsite or in home rehabilitation program for persons throughout the Greater San Diego area. Our program brings some of the most needed services right where they are most needed – we provide rehabilitation services and care coordination onsite at home.
By delivering our services in the patient's home environment - the results are clear: higher compliance, less relapse, better attendance, more complete participation in therapy and quicker recovery. For some of our patients, LifeSpan Physical Therapy Services, P.C. is likely the only realistic option - especially when travel, fatigue or other barriers prevent attendance at traditional outpatient clinic-based therapy.
Many of our patients are referred by local physicians who are looking for a way for their mobility restricted patients to benefit from therapy without having to travel beyond their abilities. Other patients are referred by community agencies that serve the senior and low mobility community. Additionally, many of our patients are referred to us from Home Health Agencies or traditional outpatient clinics (when patients struggle to attend or arrive too fatigued to participate completely).
Who Is This Program For?
LifeSpan Physical Therapy Services, P.C. is designed for the general community - especially in areas with a high proportion of senior citizens likely to benefit from rehabilitation services but who may not have ready access to those services. We treat both home bound and non-home bound patients who may not be as mobile as they wish or who may not feel comfortable traveling to a clinic for needed therapy services. Some of our patients simply prefer to be seen in the comfort of their homes.
What Are the Features of In Home Therapy?
Onsite and in home rehabilitation services by fully qualified and licensed physical therapists, occupational therapists, speech therapists, and medical social workers in coordination with you and the patient's own local physician.
How Does It Work?
Once you identify a person who has the need for rehabilitation services, simply call us at (858) 412-9349 or fax to us a referral at (619) 568-3313. A physician referral is helpful but not required (We will be happy to contact the patient's physician for the referral to therapy). As soon as we have the information, we will begin.
If the patient is eligible for home health agency services, we can give you a list of home health agencies with whom you can proceed. Our list of preferred home health agencies is based on our past experience with their staff members and their overall professionalism!
Is this Program Covered by Health Insurance?
For most senior citizen patients, the rehabilitation services are covered under Medicare Part B (not part of the Home Health Agency Prospective Payment System) with only a small co-payment by co-insurance. We always verify insurance coverage prior to initiating services.
For most of our patients, who have Medicare Part A, and qualify under the definition of homebound, our Services are covered through a home health agency!
Safeguards for Success
Depending on the specific needs of each individual patient, there may be a physical therapist, and/or occupational therapist and/or speech therapist involved in their rehabilitation program. Each of these professionals is fully licensed and experienced with geriatric care. We currently utilize our Medical Social Workers exclusively with our Home Health Contracted Agencies.
